Key Responsibilities:
Interacts with patients, family and other healthcare providers as appropriate, to ensure continuity of services and to achieve desired patient’s outcomes through safe, timely and cost-effective medication therapy. Identifies resolves and prevents medication-related problems. Monitors and recommends, when necessary, patient medication therapy and provide drug information
Job Requirements:
♦ Doctorate in Pharmacy (PharmD) plus license to practice from Ministry of Public Health and member of Syndicate of Pharmacists
♦ 1-year Residency Clinical Pharmacy (PGY1) or 2years’ experience in clinical pharmacy practice in a hospital
♦ Great organizational skills
♦ Good observation skills and attention to details
♦ Ability to work carefully, methodically and accurately with medicines
♦ Computer Literate
♦ Good knowledge of English / French.
